One Voice Consulting Limited (Confluence) is a UK digital healthcare solution SME with a core project team of Ngozi Fakeye (project lead), Gbenro Fakeye (technical lead) and Maureen Kale (industry lead). One Voice Consulting is seeking funding to develop Confluence, a secure, innovative integrated care system bringing together patient care information from all health and social care service providers. Integrating all interactions in one place will help resolve the costly issue of bed blocking when a patient in an acute or non-acute care setting is ready to be discharged but still requires some care, either at home or in a care facility but a care package is not yet in place.
