The ten year vision of the Department of Health & Social Care is boosting support to help people live at home with their families for longer and ensuring that health and care work hand in hand so people get the help they need. Care Networx is a Social Care App for families who need help in coordinating the care for a loved one with a long term health condition. The core application is to provide a permission based communication tool to care givers (family and/or carers). We then provide a layer of technology and data that sits ontop of the core functionality allowing the users of the App access and choice of integration to certified and best of breed products and services that are relevant to the long term health condition, and location specific, which aids to provide proactive care of the individual they are caring for. Our platform with be API driven enabling users to; (1) connect with wearable technology to their specific or general health care needs; (2) connect with home monitoring devices (3) bring in data from services specifically for their long term health condition and by location, such as information from the 3rd sector or local authority help. The innovation combines all the above in one simple and easy to user interface.
